Primary Care Involvement


Why PCT Involvement

Changes within the NHS have led to the need for even greater understanding between Primary and Secondary Care. The Trust has actively encouraged greater co-operation between the two sectors, particularly since the introduction of Primary Care Groups (PCG’s).

Primary/Secondary Care Links

We are involved in improving the links between Primary and Secondary Care, and we currently have one joint post between the hospital and Northampton PCT.

Within the hospital the role is as an Interface Pharmacist to help address some of the Primary/Secondary Care interface issues and promote the concept of seamless patient care. Working in both Sectors allows an invaluable insight into some of the issues from ‘both sides of the fence’!!

We have previously also had a joint post with Daventry and South Northants PCT. The hospital and the PCT are keen to continue this relationship, but we have been unable to recruit and fulfill both organisation's needs.

Structure of Pharmaceutical Care in Northamptonshire

There are 3 PCT’s in Northamptonshire. All have Prescribing Advisors and we are developing a network of these Advisors and the two Chief Pharmacists of the DGH Trusts.

A key part of this structure is the Northamptonshire Prescribing Advisory Group (NPAG).
